Before you begin
Who should fill this form
- Every community member aged 21 or above living in Mumbai — unmarried or married — must fill this form, one per person.
- Only for those whose native district is Junagadh, Bhavnagar or Amreli.
- Widowed and divorced women should fill the form separately.
Tips to fill it correctly
- Fields marked with an asterisk (*) are mandatory.
- Choose the one of Mumbai's seven Centres where you live.
- For children, list only unmarried sons under 21 (with name and age); for unmarried daughters, give the name and age.
- If you are a life member of the Mumbai Samaj, keep your receipt number, receipt date and card/member number ready.
